We accept PayPal/credit card payment. Upon completion of registration, you will receive a confirmation message with payment details enclosed. Bears Soccer Camps are run by Go! Soccer LLC.
For Camp - you must fill out the registration form (1/4). We will also need a completed medical waiver/release form (2/4), an updated immunization record (3/4), and proof of current insurance (ex. front + back of card, etc.) (4/4). Only when we have all of these forms, as well as full-payment, will the camper be allowed to participate in camp.

To avoid any problems you should keep copies of these completed forms for your records and bring them with you to registration on the first day of camp along with a copy of both sides of the camper's insurance card. Then if for some reason we do not have one or all of these forms on file the camper will be able to participate right away.
We do not provide any refunds for withdrawals/cancellations within 10 DAYS of the camp start date. All cancellations/refunds are subject to a $100 administrative fee.
